Rumah Sakit Umum Daerah Sleman - RSUD Sleman is a regional general hospital in the Sleman Regency. The Hospital is an Organizational Unit (SKPD) in the Government of Sleman and is fully-owned by the regional government of Sleman Regency. The Hospital was established in 1977 and initially registered as the Government General Hospital with type D. In 1988, the status of RSUD Sleman increased to class C. Since 2003, the Hospital's status increased to class B Non-Education. RSUD Sleman is registered at Jl Bhayangkara 48 Triharjo Sleman, Yogyakarta Special Region. General Requirements

1. Indonesian Citizen (WNI), preferably have valid ID Card Sleman;
2. Devoted to God Almighty;
3. Maximum age of 35 years old for D3/S1 and 30 years old for SMA or equivalent by 1 January 2016;
4. Have educational qualifications in accordance with the position;
5. Hand-written cover letter with black ink addressed to Direktur RSUD Sleman (without stamp duty);
6. Physically and mentally healthy as evidenced by a medical certificate from doctor who has a practice license (SIP);
7. Have a diploma from an accredited study program of at least C at the time of issued diplomas, special for SMK applicants minimum accredited B;
8. Have a good behavior as evidenced by SKCK of the Local Police Office/POLRES (submitted after passing the final selection);
9. Registered as job seekers as evidenced by the Seeker Identity Card (Kartu Kuning / AK.1) of the Local Labor Department Office (submitted after passing the final selection);
10. Affidavit does not require appointed as a permanent employee or candiate civil servant (submitted after passing the final selection);
11. Affidavit was never dismissed with no respect as civil servants or private employees (submitted after passing the final selection);
12. Affidavit was never sentenced to prison by a court decision that has had permanent legal force (submitted after passing the final selection);
13. Affidavit willing not bound contract with other parties (submitted after passing the final selection);
14. Have good communication skills;
15. Affidavit would not marry in the first year of work.

2. Curriculum Vitae (signed without stamp duty);
3. Legalized copies of diploma and academic transcripts;
4. Copy of valid ID Card (legalized from village to district);
5. Passport photograph 4x6 (2 sheets);
6. Medical certificate from doctor who has a practice license (SIP);
7. Copy of certification accreditation;
8. Affidavit would not marry in the first year of work.
